To legally be enrolled as a Falcon employee, you must complete certain government required forms.

After you have downloaded the Acrobat Reader software, you can access any of the forms below by clicking on the link in the link bar.

IMPORTANT - Please read instructions on the forms carefully. Whenever you are completing union, government or Falcon forms, please use your full legal name not your stage name.
